In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data (GDPR), I hereby inform you that the proper provision of the service requires the processing of personal data within the scope, for the purposes and under the conditions set out below:
Booking a therapy session - the processing of data is necessary to register the user for an online therapy session appointment.
Provision of the therapy service - the processing is necessary for the proper performance of the service, including contact with the client and the preparation of documentation.
Issuing invoices and meeting accounting obligations - the processing is carried out to meet the legal requirements related to the Accounting Act and tax obligations.
Maintaining contact with the client - for the purposes of ongoing communication and sending information related to the service provided.
1) Article 6(1)(b) GDPR - performance of a contract to which the data subject is party.
2) Article 6(1)(c) GDPR - compliance with a legal obligation (e.g. issuing invoices).
3) Article 6(1)(f) GDPR - the legitimate interests pursued by the controller.
The data may be transferred only to entities authorised to obtain it under the provisions of law (e.g. tax authorities) and to entities supporting the controller in conducting its business (e.g. an accounting office).
The data will be processed for the period necessary for the proper performance of the contract and for the period resulting from accounting obligations (5 years, counted from the end of the calendar year in which the last transaction took place).
The person whose data is processed has the right:
1) to obtain access to this data (Article 15 GDPR),
2) to request rectification of the data (Article 16 GDPR),
3) to request erasure of the data - the "right to be forgotten" (Article 17 GDPR),
4) to request restriction of processing (Article 18 GDPR),
5) to receive the data in a machine-readable format (Article 20 GDPR),
6) to object to the processing (Article 21 GDPR),
7) to withdraw consent at any time (Article 7(3) GDPR).
If you consider that the processing of your personal data infringes the provisions of the GDPR, you have the right to lodge a complaint with the President of the Personal Data Protection Office (ul. Stawki 2, 00-193 Warsaw, Poland).
Providing personal data is voluntary but necessary to conclude the contract and to provide the service. Failure to provide the required data will make it impossible to conclude the contract.
